Our client is a local government entity committed to delivering essential public services and fostering community development. The Intake Social Worker will serve within the Aging and Disability Resource Center, supporting clients through assessments, referrals, and case management to assist older adults, persons with disabilities, and caregivers.

This is a full-time position based in Arlington, VA, paying $28.50 per hour. This is a contract position.

Responsibilities

• Conduct comprehensive assessments of clients’ needs, strengths, and abilities to develop person-centered care plans.
• Interview clients, perform intake and assessment duties, and make referrals to appropriate assistance programs.
• Assist clients and families with completing paperwork to determine eligibility for DHS and related programs.
• Counsel clients on available options, providing guidance and making necessary referrals.
• Monitor cases for up to three months during client transitions and address any additional needs.
• Maintain accurate records of all client interactions following established standards.

Qualifications

• Bachelor's Degree in Social Work, Sociology, Psychology, Mental Health, or Counseling; Master’s Degree preferred.
• One year of experience in human services or social services program delivery.
• Experience providing clinical social work to older adults and persons with disabilities (preferred).
• Familiarity with electronic record-keeping systems such as Peer Place.
• Experience working in an ADRC or with culturally diverse populations.
• Strong interpersonal, problem-solving, and case management skills.
• Ability to work with minimal supervision.
