Our client is a local government entity committed to delivering essential public services and fostering community development. The Human Services Specialist will provide case management and support services to older adults and individuals with disabilities, helping them connect to long-term supports and resources to promote aging in place. This is a full-time, contract position based in Arlington, VA, paying $24.00 per hour.

Responsibilities

• Conduct intake and assessments for clients seeking services or emergency assistance via phone, email, or in person.
• Complete initial assessments and annual reassessments using state-wide assessment tools.
• Record and manage client information in the database system of record.
• Perform short-term case management to connect clients with necessary services.
• Document all interactions and maintain accurate records.
• Support planning and implementation of aging services such as meal programs, transportation, and caregiver supports.
• Collaborate with team members and seek guidance for complex cases.

Qualifications

• Bachelor’s Degree in Social Work, Sociology, Psychology, Mental Health, or Counseling.
• One-year experience in human services/social services.
• Desire to work with older adults and persons with disabilities.
• Experience documenting client encounters in electronic record systems.
• Ability to work with diverse populations.
• Bilingual skills in Spanish, Korean, Amharic, or Vietnamese are a plus.
