Our client is a local government entity committed to delivering essential public services and fostering community development. The Intake Social Worker will conduct client interviews, complete intake/assessment duties, and coordinate referrals to appropriate assistance programs to support older adults, individuals with disabilities, and caregivers.

This is a Full-time, Contract position based in Arlington, VA, paying $28.50 per hour.

Responsibilities

  • Conduct comprehensive client assessments to evaluate needs, strengths, abilities, and eligibility considerations for human/social service programs.
  • Develop person-centered care plans and support delivery of services ranging from information and referral resources to emergency financial assistance and shelter referrals.
  • Assist clients and families with required paperwork, including consent forms, to support program eligibility determinations.
  • Provide counseling on available options and make appropriate referrals to service providers and assistance programs.
  • Monitor cases for up to three months as clients transition into referred programs and address any additional needs identified during that period.
  • Maintain accurate documentation and ensure all client interactions are recorded in the system of record according to defined standards.

Qualifications

  • Completion of the requirements for graduation from an accredited college or university with a Bachelor's Degree in Social Work, Sociology, Psychology, Mental Health, or Counseling.
  • One-year experience in human services/social services program delivery, including counseling, training/teaching, and/or making social services referrals.
  • Preferred: A Master’s Degree.
  • Preferred: Experience providing professional clinical social work services to older adults and persons with disabilities who live independently and age-in-place.
  • Preferred: Experience documenting client encounters in an electronic record keeping system.
  • Preferred: Familiarity with Peer Place for encounter documentation.
  • Preferred: Experience working in an ADRC and with individuals from diverse cultural/ethnic backgrounds.
  • Demonstrated interpersonal skills to persuade or manage others and resolve complaints, along with strong problem-solving skills for vaguely defined practical issues.
  • Ability to complete work assignments with minimal advice or review from the immediate supervisor.
  • Ability to pass a pre-hire background check that may include criminal record, driving record, education, professional licensure, and credit history (as applicable).
