Our client is a local government entity committed to delivering essential public services and fostering community development. The Human Services Specialist will conduct intake, assessments, and short-term case management to assist older adults and individuals with disabilities in Arlington, VA. This position offers a pay rate of $24.00 per hour and is a full-time contract role.
Responsibilities:
- Conduct intake via phone, email, or in-person visits to the Aging and Disability Resource Center seeking long-term services, support, or emergency assistance.
- Complete initial assessments and annual reassessments using the Virginia statewide assessment tool, recording information accurately in the database.
- Assess individual needs and strengths, design activities to meet those needs, and connect clients to appropriate services.
- Perform short-term case management, complete necessary paperwork, and facilitate referrals.
- Document all interactions in the No Wrong Door system (Peer Place).
- Support planning and implementation of aging programs such as home-delivered meals, health promotion, transportation, and caregiver services.
- Assist other ADSD teams with phone screening and triage.
- Serve as an active team member seeking advice from clinical staff for complex cases.
- Perform mandated assessments for nutrition programs and record-keeping.
Qualifications:
- Bachelor’s Degree in Social Work, Sociology, Psychology, Mental Health, or Counseling.
- One year of experience in human services or social services program delivery, including counseling and making referrals.
- Preferred: Master’s Degree in related fields.
- Experience working with older adults and persons with disabilities who choose to age-in-place.
- Experience documenting client encounters electronically, preferably in a No Wrong Door system.
- Bilingual in Spanish, Korean, Amharic, or Vietnamese preferred.
- Strong interpersonal, problem-solving, and coordination skills.
- Ability to work effectively with diverse populations.
- Must pass pre-hire background check.
